A hospital admission moves fast, and the people making decisions about your care often meet you for the first time that day. Amy DuBois, RN, BCPA, closes that gap. She arrives knowing your medical history, your medications and your treatment history, communicates them to the hospital-based team, follows the stay from admission through discharge, and stays involved through the transition home — the point at which most avoidable readmissions begin.
What this includes
- Inform the hospital-based medical team of health, medication and treatment histories of the client
- Follow the progress of the hospitalization to improve patient/family understanding and communication with the medical team
- Assist in the post-hospitalization transition to reduce the need for re-hospitalization
- Facilitate communication to the private physicians and other related providers (therapists, rehab personnel) of the events of the hospitalization

At admission
Amy brings the hospital team a clear picture of health history, current medications and prior treatment — information that is otherwise reconstructed under pressure.

During the stay
She follows rounds and test results, translates what is being said into language the family can act on, and makes sure your questions reach the team.

At discharge and after
Amy works the transition home or to rehab, and passes the events of the hospitalization to your private physicians, therapists and rehab staff so nothing is lost in the handoff.
Common questions
- My family member is in the ER right now. Can you come?
- Call 520-909-8203. Amy has met clients at the emergency room when family could not be there, including when a spouse was out of town. Availability depends on her schedule, so call rather than email if the situation is urgent.
- Doesn't the hospital already provide a patient advocate?
- Hospitals employ patient representatives, and they can be helpful — but they are employed by the hospital. Amy is hired by you and works only for you. That independence is the point.
- Why does the transition home matter so much?
- A large share of readmissions trace back to the days just after discharge: medications changed during the stay, follow-up appointments never booked, instructions the family did not fully understand. Amy works that window deliberately.
